Seeking a Treatment Plan

What to prepare for in advance:
Try and have the appropriate medical history and insurance information handy. Doing so will ensure a smooth process for both you and the Medical Assistant who is facilitating you.

The following will be helpful prepare:
How long the substance use has been going on?
How much of the substance is being used consistently? 
If any other drugs are being abused at the same time?
If there are any known issues/diseases?
If other mental health disorders are present?
